Reducing Infant Mortality

Getting to 1, the Ohio Equity Institute’s Lucas County team working to reduce infant mortality, has partnered with ProMedica, Mercy Health and the University of Toledo Medical Center to launch an assessment and referral project in OB/GYN practices. Pregnant women with risk factors for poor birth outcomes or infant death – including a lack of food, housing, transportation and other issues – are referred to the Northwest Ohio Pathways HUB so they can be connected to community resources.

Additionally, the Northwest Ohio Pathways HUB recently received a grant from the Ohio Commission on Minority Health to help expand and replicate the Pathways Community HUB Model to reduce infant mortality statewide. The Pathways team will serve as mentors for another Ohio community launching a HUB.

Reducing infant mortality is one of Healthy Lucas County’s five priority health areas. Below is an infographic about low birth weight, the leading cause of infant mortality in Lucas County.
