Get Help from Pathways

Pathways is known for helping pregnant women deliver healthy babies by getting needed medical care and social services. Now Pathways has expanded to help adults with diabetes and heart conditions or who are at risk for these chronic diseases.

Community health workers help Pathways clients get access to needed medical care and social services. This includes getting health insurance coverage, diabetes education, smoking cessation classes, food, housing, transportation, clothing and more.

Pathways is enrolling adults with diabetes, high blood pressure and heart disease. We also are enrolling adults who are at risk for these chronic conditions.
